FDA has approved two products for use in human medicine:
Vowst is capsules of human stool-derived spore-forming microbes developed by Seres Therapeutics and commercialized by Nestle.
Rebyota is a simple FMT enema, developed and commercialized by Swiss multinational Ferring Pharmaceuticals.
The National Institute of Standards and Technology has created a product consisting of vials of fecal slurry from screened human donors, as a reference tool for research purposes
Probiotic Infusions products are NOT FDA-approved, or intended for use in preventing or treating human disease. They can be used for research or other non-medical purposes.

Donor screening is a rigorous process. We start by identifying people with slender or athletic body types, and no health conditions that could be related to the microbiome (including but not limited to current or past chronic gastrointestinal disease, autoimmune disease, asthma, atopic dermatitis, diabetes and metabolic syndrome, mood disorders, overweight or obesity, cancer, and certain types of chronic pain)
We test those healthy people’s gut microbiomes using whole-genome sequencing. Unfortunately, many otherwise very healthy people get eliminated as donors at this phase!
Those people with the best microbiome results get tested with additional blood and stool tests, to rule out HIV, syphilis, viral hepatitis, common enteric pathogens, ova and parasites, MDRO testing, and C diff.
Donors have not had antibiotics in at least six months before donating, generally much longer. They have been screened for multi-drug resistant organisms.
Donors are re-screened every three months and are excluded in cases of recent international travel and acute illness.

FMT products need to be kept at a constant temperature, which is why home freezers aren’t a great place to store them; home freezers go through automatic thaw/re-freeze cycles so ice doesn’t build up n the inside. If you’re planning to store FMT products for weeks or months, a -20C or colder medical freezer will work. If you’re planning to store them for a shorter period of time, they can be kept in a cooler with dry ice ON TOP of them. In most circumstances, 5 lb of dry ice will keep the product at a safe cold temperature for ~24h, so if you want to store FMT products for four days, you could use 20lb of dry ice. However, the thickness of cooler walls, ambient temperature etc. can change dry ice needs, so check how much dry ice is left in your cooler at least once per day.
